Effect of Vitamin D Supplementation in Children with Severe Acute Malnutrition

Background: Malnutrition accounts for about 35% of under-five mortality, of which 4.4% is specifically because of severe wasting. Prevalence of vitamin D deficiency in malnourished children ranges from 31% to 61%. Addition of high dose vitamin D supplementation in children with severe acute malnutrition would be effective in elevating serum vitamin D concentration and thus improve weight gain. The association of vitamin D and malnutrition is well recognized but data are scarce. Objectives: To see the effect of vitamin D in the treatment of severe acute malnourished children. Methods: This prospective randomized controlled trial study was conducted among children 6 months to 5 years old with a diagnosis of SAM in the pediatric ward of Dhaka Shishu (Children) Hospital in Bangladesh from July 2018 to January 2020.Sample was collected through simple random sampling then randomized into two groups by lottery method. Finally, 50 patients were included in the study, among them 25 patients received 6 lac IU vitamin D orally in day 1 in addition to the standard treatment of SAM and the other 25 patients received only the standard treatment of SAM. Demographic data and other related information regarding clinical feature, laboratory feature, outcome variables were recorded and anthropometric measurement and Z-scoring was done to assess malnutrition. Collected data was checked manually and analyzed by computer-based program SPSS for Windows (version 20.0). Outcomes were: Length of hospital stay and average weight gain between the two groups. Comparison of parameters among the group were done by unpaired t-test and chi-square test. Results: There were no statistically significant difference in length of hospital stay (16.24 ±3.29 versus 15.34±2.64) days, p=0.291 but there was statistically significant difference in average weight gain (9.08±2.99 versus 7.52±2.06) gm/kg/day, p=0.037 and wasting (weight for height z score) improved significantly on follow ups (p=0.241 on first, p=0.039 on 2nd, p=0.009 on 3rd and p=<0.001 on 4th follow up). Conclusions: Vitamin D supplementation causes significant weight gain in children with severe acute malnutrition.

Factors affecting Early Discontinuation of Exclusive Breastfeeding – A study of Hospitalized Infants

Introduction: Breast milk is a natural and nutritious food which is considered to be the safest food for neonates and infants. It boosts immune system, controls gastrointestinal infections, pneumonia, UTI and provides proper nutrients like protein, fat, vitamins, and minerals. Aim of the Study: The study aimed to find out the factors of early discontinuation of exclusive breastfeeding (EBF) during the first 6 months of life of hospitalized babies. Methods: It is an observational prospective study conducted at the Bangladesh Shishu Hospital and Institute. The sample size was 191 infants and their mothers. Data was collected through a structured questionnaire prepared by the researchers and applied to the mothers of infants. The study was conducted during the period of 1 July 2020- 31 December 2020. Result: Most of the infants 111(52.3%) were aged<20 days and most of the mothers 154(80.6%) were aged more than 20 years. Majority of the mothers 107(56.1%) were housewives and 14(7.3%) infants were preterm. Most of the babies 125(65.4%) weighted more than 2500g. In this study majority of the babies 161(84.29%) were healthy cried immediately after the birth. Majority of the babies 105(55%) could suck breast milk properly after the birth. In 103(53.9%) cases, breastfeeding was initiated within an hour after the birth. Normal vaginal delivery was done in 98(51.3%) cases and caesarean section was done in 93(48.7%) cases. Majority of the mothers113 (59.2%) had misconception about quantity of milk they believed that they don’t have enough milk. Only 32(16.8%) mothers doesn’t like breastfeed the baby and 65(34%) mothers believed that formula is better than breast milk and 107(56%) mothers had lack of knowledge about advantages of breast milk. Conclusion: This prospective study found that the level of the discontinuation of exclusive breastfeeding is high having several factors like having misconception about quantity of milk, lack of knowledge about advantages of breast milk, preference of formula milk over breast milk. More over low socio-economic background, less educational level of mothers, working mothers having no breast feeding corner in work place, and mothers having various breast problem (inverted nipple, flat nipple, cracked nipple, Mastitis, breast abscess, Faulty technic of breast feeding ) were also included.

The Causative Organism of Neonatal Sepsis by Blood Culture

Background: Neonatal septicemia is a clinical illness that occurs within the first four weeks of life and is defined by systemic signs and symptoms brought on by a widespread bacterial infection. The most frequent cause of illness and mortality in the neonatal era is bacterial infections. Early detection of this potentially fatal illness is essential for prompt treatment and a successful outcome since complications like shock, disseminated intravascular coagulation, and multi-system organ failure can lead to the fulminant and lethal course of infection. Objective: The present study was undertaken to detect the causative organism of neonatal sepsis by blood culture. Method of the Study: This is a cross- sectional study carried out in the Department of Neonatology, BSMMU, Dhaka over a period of 6 months between January 2013 to June 2013. Newborns with suspected sepsis admitted to BSMMU were the study population. Clinically diagnosed cases of neonatal sepsis aged < 28 days of both sexes whose parents or guardians provided informed consent were eligible for enrollment in the study. The subjects were selected consecutively from the study population. Data were collected using a structured questionnaire containing all the variables of interest. Data were processed and analyzed using the computer software SPSS (Statistical Package for Social Sciences). The test statistics used to analyze the data were Chi-square (χ2) or Fisher’s Exact Probability Test & Student’s t-Test. Result: The study included a total of 138 neonates (both preterm and term) of clinically diagnosed cases of sepsis. All the neonates were subjected to test for WBC, IT ratio CRP, and blood culture. In terms of gestational age over 80% were preterm and the rest 19.6% were term neonates. Among the neonates, 40 (29%) had early onset sepsis (EOS) and 98(71%) had late-onset sepsis (LOS). Of the culture- positive cases, Klebsiella was predominant (33.33%) followed by E. coli (22.2%), Acinetobacter (16.66%) and Pseudomonas (11.1%). Less commonly found organisms are Staphylococcus aureus, Enterobacter, Enterococcous Cytrobacter, and others. Conclusion: Sepsis screening is more useful for ruling out the diagnosis of newborn septicemia, which can be made rationally if two screenings conducted 12 to 24 hours apart are negative. It is preferable to wait for the findings of the sepsis screen before starting antibiotics in a neonate who is otherwise stable or suspected of having sepsis due to maternal risk factors.
